We service Gympie, Kingaroy, Sunshine Coast and surrounding areas. We do home visits for NDIS, Nursing care and DVA clients. In addition, we provide telehealth services via Zoom, Facetime, Skype or over the phone for Care Plan (EPC) and private clients.

NDIS

Our experienced practitioners can work with you to develop a treatment plan and provide a quote for your plan manager.

DVA

We accept referrals for all clients with DVA Gold Cards. No referrals necessary if you are not accessing DVA.

Bulk Billing

We offer a Bulk Billing option for clients referred through the Chronic Disease Management plans (also called the Enhanced Primary Care (EPC) scheme).

Medicare

We are keen to participate in Chronic Disease Management plans (also called the Enhanced Primary Care (EPC) scheme), to enable clients to have no out of pocket expenses. Your GP can create a plan for you if you are eligible. No referrals necessary if you are not accessing Medicare.

Nursing Care

Our experienced practitioners in aged care are able to support and guide clients to their nutritional goals.

Referrals

We are registered with medical objects, so GP clinics can send referrals securely and directly from most practice software packages. Just search in medical objects for Jacquie Kaye.
